Please read the Pavilion and Church Grounds Rules, Regulations, and Code of Conduct before submitting your request

GENERAL GUIDELINES

  1. Persons/Organizations who desire to use the Pavilion and/or grounds need to complete the Fairview Avenue Church Pavilion Usage Request Form. Fairview Avenue Church staff will process the Reservation Form and upon approval of the request will schedule the activity. 
  2. Scheduling of events will be based on the availability. The requesting person/organization is responsible to begin and end their activity at the times requested and approved.
  3. The person completing the Reservation Form must be present during the entire event.
  4. Any damage that may occur during use will be the responsibility of person/organization using the facilities. 
  5. The person/organization using the Pavilion and/or grounds is responsible for leaving the area clean and in the condition they found it.
  6. Fairview Avenue Church will not be responsible for any injuries or accidents that occur on the premises. In case of an emergency Fairview Avenue Church does not provide first aid or telephone service. We strongly recommend that you bring a first aid kit and cell phone to your event.
  7. All accidents must be reported within 24 hours in writing to the Fairview Church Office.
  8. All trash and consumables must be removed and deposited in the provided garbage containers.
  9. Decorations may not be attached to pavilion using screws, nails, staples, etc. All decorations must be removed from the building immediately after the event.
  10. Pavilion uers may grill on adjacent grass area provided that grills are located no less than 6 feet from the structure.
  11. A Facility Cleanup Checklist must be completed before leaving the facility at the conclusion of the event.
  12. Fairview Avenue Church reserves the right to terminate the event and/or bill for damages if provisions of the guidelines are violated.
  13. Weddings must be arranged through the pastor and according to church guidelines.

CODE OF CONDUCT

  1. Parents are responsible for the supervision of their children while using the Pavilion/Play Area.
  2. All music played at Pavilion must be appropriate (no foul language) and played at a non-offensive level.
  3. No alcoholic beverages, tobacco products, illegal drugs, fireworks, or unauthorized weapons will be allowed at the Pavilion.
  4. Profanity and disorderly conduct are not permitted.
  5. Unauthorized motor vehicles, including motorcycles, scooters, and 3 or 4 wheeled ATVs) are prohibited from grass areas at Fairview Avenue Church.
  6. Riding of skateboards, bicycles, scooters, roller skates, etc. is prohibited inside the pavilion.
  7. Dogs are to be leashed at all times. Please clean up after your dog/pet.
  8. Picking of flowers or cutting or damaging any plant life is prohibited. No plant or animal life shall be removed from Fairview Avenue Church unless authorized. For weddings this includes not throwing rice, birdseed, releasing doves or balloons as these all have a negative environmental impact.

COVID LIABILITY

An inherent risk of exposure to COVID-19 exists in any public place where people are present. COVID-19 is an extremely contagious disease that can lead to severe illness and death. Persons/organizations using the Pavilion assume all risks related to exposure to COVID-19.
